What is Targeted Case Management?

Targeted Case Management (TCM) is a comprehensive support service designed to help individuals navigate their mental health treatment journey effectively. Our skilled Targeted Case Managers work closely with clients to develop and achieve personalized goals and objectives.

Our TCM services are offered FREE of charge to qualifying Medicaid recipients.

Daven Mansfield, BS​ - Program Director, Targeted Case Manager

Sometimes life’s challenges can be overwhelming. I believe that despite those challenges, everyone deserves the opportunity to live a happy, healthy, satisfying life. All too often the life we envision requires resources and opportunities that are difficult to identify and navigate. My position allows me to offer a helping hand. As a targeted case manager, I am experienced in providing services including finding employment, obtaining disability or SNAP benefits, behavioral health, medical, and medication management referrals and appointment management, food and housing assistance, educational needs, and much more! By working with the client to identify areas of need and develop agreed-upon goals to address those needs, clients will develop skills and utilize resources integral to living the life they envision.

Susie is an experienced elementary school teacher with a master's in Special Education from Eastern Kentucky University. For the past 5 years, she has dedicated herself to teaching 5th-grade reading. Before this, Susie spent a decade at Berea College's Child Development Lab as an early childhood teacher. Now, driven by her passion for serving children and families, she seeks to expand her impact within the community.

I have been working in the field for the past 8 years and have also provided individual therapy. Professionally, my passion for social justice and instilling confidence and self-efficacy are foundational elements for working with my clients. I work with both children and adults with issues ranging from substance abuse to linking life-enhancing and sustaining resources. In my free time, I enjoy spending time with my wife Daven and our three wonderful kids, and rooting for the Reds, Bengals, and Wildcats!
